I acknowledge and agree to the following:

• 2820763 Ontario Inc. o/a Guardian Rescue Services (“the Company”) may offer me work hours in excess of 8 hours per day and 48 hours per week up to a maximum of 60 hours per week and I agree to work such hours as are scheduled. I understand that this Agreement is permitted by the Employment Standards Act, 2000.
• I have received and read the most recent information sheet entitled: “Information for Employees about Hours of Work and Overtime Pay” as produced by the Director of Employment Standards for Ontario.
• I understand that I may revoke this Agreement by providing the Company with two weeks’ written notice of revocation.
• I understand that the Company may revoke this Agreement by providing me with reasonable notice.
• I acknowledge having read and understood this Agreement and I have not been in any way pressured or coerced into signing it.
